Our personal/home care division providesan individual with non-medical/personalized assistance that allows them to remain in their home. Our licensed and bonded caregivers can provide additional support services to you during your recovery and even after discharge.

Whether it’s few hours a day or all day, every day, our services are specifically tailored to meet the client’s needs. While the clients we serve are mostly in Alameda and Contra Counties, we do not mind extra mile for services like doctor’s appointment, events and errands. Because some clients may require less extensive help, we also offer Housekeeping and Chores, Handyman, Errand Runs, “Bed and Breakfast”.

PERSONAL CARE TEAM

Director of Client Services

Director of Client Services makes the first appointment, assesses the client and families need, develops care plan and finds matching caregiver. He/She then maintains regular oversight on the care, communicates with family members and coordinates care with home health if there is a need.

Scheduler

Manages schedule for the client-caregivers. Makes amendments when necessary and communicates with client, caregivers and Director of Client Services.
Caregivers: Provide high quality, compassionate and supportive care services to the clients in their home. Communicate status updates to Director of Client Services.

RN Case Manager

This licensed and trained professional coordinates and manages the patient’s care, evaluates their condition and needs, and implements the Physician’s treatment plan.
